INSURANCE CARD(S) (FRONT AND BACK) - our office must have a copy of your current primary AND secondary (if applicable) insurance cards PRIOR to your visit. Failure to provide these items ahead of time will result in the rescheduling of your appointment.

A copy of your card(s) can be submitted prior to your appointment by any of the following methods:

Upload a of your card(s) to your MyChart patient portal: epic.mycenturahealth.org

Upload a copy of your card(s) during eCheck-In for your upcoming visit. It is REQUIRED eCheck-In be complete at least 48hrs prior to your visit. eCheck-In is done within your MyChart patient portal: epic.mycenturahealth.org

Text a picture of the front and back of your card(s) to your scheduler (they will provide you the number when scheduling your appointment), or text the pictures directly to (303) 226-6732.

Email a picture of the front and back of your card(s) to frontdesk@cpimedicine.com.

Bring your physical card(s) to our office and we will make a copy.

Our company policy requires we have a copy of your insurance card(s) PRIOR to your appointment. If you choose to bring your cards with you to your visit but forget, your appointment will be rescheduled. We highly recommend you submit a copy of your card(s) ahead of time to avoid appointment cancellations/reschedules.

MyChart is CPI’s patient portal. All patients seen at CPI are required to have an active MyChart portal account. Your portal will be used for many things, including but not limited to:

MYCHART PATIENT PORTAL

All patients must have an active MyChart patient portal account. The portal is how the doctors will be communicating with you, how you can get medications refilled with a click of a button, view test results, view your medical information across multiple providers, pay your bill, self-schedule and much more. Not only is the portal a benefit to you as a patient, but also to your medical providers. It is critical that your medical information in the portal is up to date and maintained to ensure you are getting proper treatment and make the most of your visit with us. eCHECK-IN (Visit Registration)

All patients must complete registration for their visit at least 48 hours prior to their appointment. This visit registration process is called “eCheck-In” During eCheck-In, you will update your medical information, medication list, demographics, insurance information and pay your co-pay. Failure to complete eCheck-In prior to your visit may result in the cancellation/rescheduling of your appointmen.

OUTSIDE MEDICAL RECORDS

When booking your appointment, the scheduler will ask you to verify your PCP, any other specialists you currently see, recent hospitalizations/ER visits, and recent testing that you've had done. We will do our best to obtain all relevant medical records based on this information you provide. If you intend to bring a copy of any records to the visit yourself, please let the scheduler know. It is critical we have recent medical records to make the most out of our visit with you. If we are unable to obtain any records we will ask you to contact your provider and assist us.

PULMONARY FUNCTION TESTING
(PFT) VISITS

In addition to the items mentioned above, patients coming in for a PFT test
must adhere to the following:

1

No nicotine (in any form) or caffeine 4 hours prior to your test.

2

No short-acting inhalers 4 hours prior to your test. Examples: ProAir Digihaler, ProAir HFA, ProAir RespiClick, Proventil, Ventolin, Albuterol, Xopenex

No long-acting inhalers 12 hours prior to your test. Examples: Breo, Advair, Anoro, Serevent, Stiolto, Spiriva, Trelegy

4

Eat a LIGHT meal 1-3 hours prior to your test.

How To Prepare for Your Sleep Study:

We will need access to your scalp to place EEG sensors (brainwave monitoring). Please no wigs, caps, or toupees. Hair should be recently washed & dry. Skin should be clean and free of makeup and lotion.

If you can, please remove artificial nails/fingernail polish from at least 2 nails. These items can disrupt readings of your oxygen saturation levels.

Please avoid caffeine, excessive alcohol, foods that cause discomfort, and naps the day of your study. Take normal medications as prescribed.

If you normally shave your facial hair, please do so prior to your study.  Full beards are exempted from this.

Good Things to Know:

1

You will be able to easily use the restroom during your study.

2

If you plan to take sleep medications, and you are concerned about how it will affect you, please arrange for transportation home.

We DO NOT have a shower/toiletries available.

4

The sensors are non-invasive/painless.

As an outpatient facility, we DO NOT have any type of nursing care available. You need to be able to perform daily living activities independently (i.e. getting in and out of bed, changing clothes, or using the restroom). If you need additional assistance, please contact the sleep lab to discuss options.

If you have problems with pain during the night, please bring your usual medications to treat this. We DO NOT have any medications available here for you to take.

If you are diabetic bring your required medications. We also encourage you to bring a snack, if needed.

If you are using oxygen, bring a sufficient amount for transportation to and from the sleep study. While you are here, we do have concentrators available for you to use.

Phillips Respironics recently issued a voluntary recall of most PAP machine models due to health concerns related to the breakdown and off-gassing of VOCs from the sound abatement foam.  Our lab unit is part of the recall.  We have removed the sound abatement foam from our units to ensure our patient’s safety and will comply with all future recall instructions issued by the manufacturer.

What to Expect After Your Sleep Study

A Polysomnographic Technologist (Sleep Tech) will connect several sensors that record your brainwaves, breathing, and muscle movements throughout the night. Despite all the sensors, most people say it doesn't significantly disrupt their sleep.

The sleep technologist will monitor you from a computer in a different room. The technologist will also help you get up during the night if you need to use the restroom and assist with any other requests.

If you are being tested for sleep apnea, you may undergo a "split-night" test. During the first half of the night, we will diagnose your sleep/breathing problem, and the other half will be used to determine the best way to treat the problem. Sometimes, you may be asked to return for another sleep study to determine the best way to treat your sleep apnea.

If PAP is needed, we will fit you with various masks and find the right fit for you. There are many options.

What to Expect During Your Sleep Study:

You will need to schedule and attend a follow-up appointment with your provider to review results. You must do this no earlier than 10 days after your sleep study, but no later than 30 days. This window allows us to meet insurance/durable medical equipment guidelines for coverage, if needed.

If PAP or oxygen is prescribed at this follow-up visit, your provider will go the next steps in how to obtain these items.
